SessionNote BH turns a counseling session into a draft progress note that clinicians can trust. Every statement cites the transcript line it came from, nothing is inferred that the session didn’t say, and the clinician reviews, edits and signs. Prepared in response to Weber Human Services’ RFB 09-14-2026, AI-Assisted Clinical Note Writing System.

What it does
WHS / Medicaid note, with evidence
Drafts the WHS / Medicaid progress note by default (service details, goals addressed, interventions, client response, progress, plan and attestation), with DAP, BIRP and SOAP as alternatives. Click any citation to see its source line.
Live language checker
Re-runs on every edit and flags uncited statements, misquotes and unsupported numbers, medicalized or diagnostic terms, and stigmatizing substance-use language, with person-first alternatives.
Clinician sign-off gate
The system never assigns a risk level: the clinician makes a required risk determination and completes mental status. Signing is blocked until every flag is resolved, and pre-licensed clinicians route to supervisor co-signature.
Finish & copy into Junction
Standalone by design. After sign-off, copy the note to the clipboard or download it as .txt or .pdf, then paste it into Junction. There is no interface to build or maintain.
Privacy built in
Client consent before capture, with dictation or a manual note if the client opts out. Role-based, minimum-necessary access, and a hash-chained audit trail that fails verification if any event is altered.
How it works
A six-step new-note wizard. Each step unlocks when its prerequisites are met.
STEP 1
Client & session
Pick the session from the schedule, choose the note template and review the client’s treatment-plan goals.
STEP 2
Consent & capture
Record the client’s consent (and a guardian’s, for a minor) before capture. If they decline, switch to dictation or a manual note.
STEP 3
Draft & review
The cited draft appears beside the transcript and the audio is deleted. Edit sections inline while the checker re-runs live.
STEP 4
Risk & mental status
Clinician-only and required. Cited risk statements sit next to the clinician’s determination; mental status is a structured clinician form.
STEP 5
Checks & sign-off
Only the rendering clinician can sign, once every flag is resolved. Signed notes are locked, and final sign-off deletes the transcript.
STEP 6
Finish & copy
Copy the signed note or download it as .txt or .pdf and paste it into Junction. Part 2 notes carry the redisclosure notice.
Privacy commitments
HIPAA BAA and Part 2 QSOA
A Business Associate Agreement and a 42 CFR Part 2 Qualified Service Organization Agreement, both signed at award.
Nothing kept that isn’t needed
Audio is deleted once it is transcribed and drafted, and the transcript is deleted at sign-off. Every deletion is logged.
No training on client data
Client data is never used to train models, contractually, and speech-to-text and drafting subprocessors are held to zero data retention under BAAs.
US hosting, cloud or on-prem
A US-hosted cloud deployment or the client’s own infrastructure on-premises, with HIPAA Security Rule safeguards, TLS 1.2+ in transit and AES-256 at rest.
Drafting in the prototype is deterministic and rules-based, and nothing leaves the browser. A production drafting model would run behind the same citation checker, sign-off gate and audit trail.
